School Registration
1. Registration for continuing families opens the last week
of January each year.
2. Registration for new families begins March 1 and continues until classes are full (20-25
students). Classes close when the maximum number is reached.

1) CERTIFIED, ORIGINAL BIRTH CERTIFICATE
2) BAPTISMAL CERTIFICATE, IF CATHOLIC
3) ACADEMIC RECORDS (MOST RECENT REPORT CARD, TESTING INFORMATION)
4) SOCIAL SECURITY NUMBER
5) STATE OF ALABAMA IMMUNIZATION CERTIFICATE (BEFORE SCHOOL STARTS)
6) COPIES OF DIVORCE DECREES AND/OR CUSTODY JUDGEMENTS
7) PAYMENT OF $75.00 NON REFUNDABLE REGISTRATION FEE PER CHILD
7) AGE REQUIREMENTS
PK3 Age 3 by September 10 of current year
PK4 Age 4 by September 10 of current year
Kindergarten Age 5 on or before September 10 of current year
First Grade Age 6 on or before September 10 of current year
ACCEPTANCE
1) REGISTRATION FORM COMPLETED AND RETURNED
2) ALL FINANCIAL OBLIGATIONS HAVE BEEN MET.
3) BIRTH AND BAPTISMAL CERTIFICATES RECEIVED
4) ACADEMIC RECORDS EVALUATED (COPY OF MOST RECENT REPORT CARD)
5) INTERVIEW WITH PRINCIPAL
Books and Supplies
· Parents must purchase required books and supplies for their children
· Book/Supply lists are published each year by May
· Students are required to have all books and supplies by the
first day of school
Tuition Assistance
· Tuition Assistance is based on need, student academic record
and behavior
· Applications should be submitted in January/February for the
next school year
· Applications can be submitted to Heart of Mary or to the Catholic parishes where the family worships
